Payment
Our office requires the payment of deductibles, co-pays and past-due account balances at the time of service. If you do not have insurance, full payment is expected at time of service.
Our offices accept cash, checks, Visa and MasterCard.
Insurance
Our physicians participate in most insurance plans. It is the patients' responsibility to know their insurance coverage and if their physician participates with their plans. Not all of our physicians participate in the same plans. We recommend you call your insurance company or check below to verify that we participate with your plan.
Our office will submit claims on your behalf for most insurances companies. Please bring your insurance card with you to every visit so we can ensure we have accurate information on file. Please notify us if you have any changes in your name, address, phone number or insurance plans.
